Going Diving? We provide PADI medical certificates for diving courses.

PADI medical questionnaire forms are available at the clinic or you can download your PADI Medical Questionnaire.

Driving Medicals

We offer a full medical check for the renewal of the New Zealand Drivers Licence (DL9 certificate).

Land Transport Safety Authority medical examination forms are available at the clinic or you can download one from the LTSA website.

Time to complete medicals

Most reports will be completed during the consultation. Allow half an hour for your diving or driving medical appointment.

What to wear

We recommend that you wear skirts and trousers, together with normal underwear or boxer shorts. Don’t forget to bring any prescriptions glasses or contact lenses.
