It is free to enrol in our medical clinic and in the Procare Primary Health Organisation.

To apply early for enrolment funding prior to your first visit to us, you will need to:

1. Download the Enrolment PDF form
2. print and sign
3. then post or email it to us TOGETHER WITH a copy of:

• Your New Zealand Birth Certificate,
• Your Passport,
• Visas or permits,
• or for Australians, other supporting evidence of eligibility.

We regret that we will not be able to process your application until we have received all the appropriate documentation.
